Listed below are the problems that have frequently occurred at the time of application. Please check your application form and document before submitting.
● Only one photo is attached.
You need two photos: one attached to the application form and the other.
● No mail address is included.
The Office will contact you via e-mail. You need to fill your e-mail address in the form.
Please provide your PC mail address, not your mobile phone, if possible. If your mail address is long and the specified column is too small to write, use the separate sheet of paper to write it in a readable way.
● Third party testimony is not filled in appropriately.
Third party testimony column is on the lower left of the application form. Your own signature needs to be placed on the lower right of the application form. We will not accept the form with your signature in the third party testimony. The person other than the applicant must sign in the third party testimony.
Please check the signature date because the date must be before or on the application date.
● Your name, performance pieces, and the date of recording are not stated on the DVD.
Please check that your name, performance pieces and the date of recording are stated on the DVD.
● No official document showing your age such as a passport (a copy allowed) is not attached.
We ask you to send some official document that we can check your age. Please send us an official document like a passport.
* Please write the application form in an easy-to-read manner.
If you can not write all information in the application form, please use a separate sheet of paper (A4-size paper) or type with a computer.
